Allows individuals to petition superior court to change their gender and sex identifier to female, male, or nonbinary on birth certificates, marriage licenses, and confidential marriage licenses, including for minor and adult children.

Permits individuals to obtain new vital records directly from the State Registrar or county clerk without a court order by submitting an affidavit under penalty of perjury that the change conforms to their gender identity and is not fraudulent.

Enables petitions for gender and sex identifier changes even if the person does not reside in California, if seeking to change documents issued by the state, with venue in the county where the birth or marriage occurred.

Requires new vital records to supplant the original records, with original documents sealed and available only by written request of the registrant or court order, with no indication the new record is not original.

Adds assigned sex at birth to confidential information collected in electronic birth registration files, accessible only to authorized entities for health-related or specified purposes; provisions become operative January 1, 2023.
