Requires the State Department of Health Care Services to convene a stakeholder group by December 1, 2023, to create a model care coordination plan for individuals discharged from temporary psychiatric holds or conservatorships.

Mandates that individuals released from psychiatric evaluation and treatment receives a care coordination plan before discharge, including a scheduled first appointment with a behavioral health provider, with no extension of detention based on plan completion.

Requires health plans, mental health plans, primary care providers, and other referred providers to make good faith efforts to contact discharged individuals at least three times via email, telephone, mail, or in-person outreach.

Directs all facilities designated by counties for psychiatric evaluation and treatment to implement the model care coordination plan by August 1, 2024.

Clarifies that counties may fund these mental health services using Mental Health Services Fund revenues and specified Local Revenue Fund sources, provided they are included in county mental health plans.
