Amends Probate Code Section 4615 to clarify that "health care" includes care to maintain, diagnose, or otherwise affect a patient's mental health condition, in addition to physical health conditions.

Amends Probate Code Section 4617 to explicitly include mental health conditions in the definition of "health care decision," specifically adding language that approval or disapproval of diagnostic tests, surgical procedures, and medication programs extends to mental health conditions.

Revises the statutory advance health care directive form (Section 4701) to clarify throughout that individuals may include instructions relating to mental health conditions, including in Part 1 (power of attorney for health care) and Part 2 (instructions for health care).

Allows persons executing advance health care directives to give instructions and grant decision-making authority to agents specifically regarding mental health treatment alongside physical health treatment decisions.

Approved by the Governor on June 20, 2022, with no fiscal impact or appropriation required.
