Requires County Health Initiative Matching Fund applicants to provide continuous eligibility for children under 5 years of age until they reach 5 years old if not determined eligible for Medi-Cal, with exceptions specified in Section 14005.255.

Implementation is contingent on obtaining federal approvals, legislative appropriation of funding with determination that ongoing General Fund resources are available in fiscal year 2024–25 and beyond, and department confirmation that systems have been programmed to implement the continuous eligibility provision.

Directs the department to issue a declaration certifying when all implementation conditions are met and post it on its website and provide copies to state officials; the continuous eligibility requirement becomes operative on January 1, 2025, or the date certified by the department, whichever is later.

Removes citizenship and immigration status requirements from the County Health Initiative Matching Fund eligibility criteria.

Provides that if the Commission on State Mandates determines this act imposes state-mandated costs, reimbursement shall be made pursuant to Part 7 of Division 4 of Title 2 of the Government Code.
