Requires death certificates to list parents' current first and middle names, birth last names, and birthplaces without reference to gendered relationship to the decedent.

Mandates the State Registrar electronically capture (but not display on the hard copy) the parents' relationship to the decedent and any additional last names used by parents.

Allows recording of a decedent's sex/gender identity as female, male, or nonbinary based on the informant's report or official documents (birth certificate, driver's license, court order, passport, etc.).

Establishes procedures for resolving disagreements about the decedent's gender identity among persons with equal rights to control disposition of remains, including potential court petition.

Requires implementation of these changes by July 1, 2024.
