Creates the Community Health Navigator Program effective January 1, 2023, to provide direct grants to qualified 501(c)(3) community-based organizations for Medi-Cal outreach, enrollment, retention, and access activities.

Grants allocated based on estimates of unenrolled eligible individuals, people of color, immigrants, limited-English speakers, homeless individuals, seniors, people with disabilities, formerly incarcerated persons, LGBTQ+ individuals, and those with mental health or substance use disorders in applicant service areas.

Department authorized to contract with private foundations to administer grant applications and allocations, and with nonprofit legal services providers to provide training and technical assistance to grantees on Medi-Cal eligibility and enrollment processes.

Requires department to publish grantee lists and amounts annually, progress reports quarterly, and conduct independent evaluations every three years.

Department must coordinate with Covered California and participating counties on joint marketing, enrollment strategies, and quarterly meetings; must report by July 1, 2023 on feasibility of a single unified navigator grant program.

Bill becomes operative only upon express appropriation in the Budget Act or another statute.
