Amends Section 11019.11 of the Government Code to clarify that state agencies requesting veteran status must use the standardized format "Have you ever served in the United States military?"

Adds Section 11019.12 requiring nine specified state agencies and California Community Colleges to include military affiliation questions on intake and application forms at their next scheduled update, asking both about personal service and whether the applicant is a family member of a service member

Requires agencies to obtain written consent from military-affiliated individuals to transmit their name, email, mailing address, and phone number to the Department of Veterans Affairs for benefits notification, with consent valid for 12 months

Mandates agencies transmit collected military affiliation information to the Department of Veterans Affairs at least annually, with data restricted to assisting individuals in accessing veterans benefits

Requests the University of California Regents to voluntarily comply with these provisions if they adopt a resolution consenting to do so
