Prohibits state agencies, local governments, and other governmental authorities from requiring individuals to provide COVID-19 vaccination documentation as a condition of receiving services or entering places.

Prohibits any public or private entity receiving state funds through grants, contracts, loans, or other means from requiring vaccination status documentation as a condition of service or access.

Applies only to COVID-19 vaccines administered under federal emergency use authorization.

Establishes that vaccination status is private health information that should not be compelled for disclosure by governmental entities.

Provides for state reimbursement to local agencies and school districts if the Commission on State Mandates determines the bill imposes state-mandated costs.
