Dissolves the Los Medanos Community Healthcare District effective February 1, 2022, with the County of Contra Costa becoming the successor to all district assets, rights, and responsibilities.

Requires the county to form a Los Medanos Health Advisory Committee and develop a Los Medanos Area Health Plan Grant Program focused on comprehensive health-related services in the district's territory.

Mandates the county complete a property tax transfer process to redirect the district's health-related ad valorem property tax revenues to the county for sole funding of the health plan grant program.

Requires the county auditor-controller to establish a special fund to track receipt and disbursement of the transferred ad valorem property tax revenues.

Directs that any new revenues generated from changes to district facilities or property be used to fund the Los Medanos Area Health Plan Grant Program.
