Establishes the Resilient Economies and Community Health Pilot Program ("outREACH Program") to be administered by the Strategic Growth Council from January 1, 2022, through December 31, 2026.

Provides grants to eligible community-based organizations to deliver coordinated incentives and services to disadvantaged communities at the household level for economic savings, greenhouse gas emissions reductions, air pollution reduction, and climate resilience.

Prioritizes awards to areas in the 91-100 percentiles of CalEnviroScreen and requires coordination of incentives including utility rebates, weatherization, electric vehicle incentives, solar installation, public transit support, and disaster preparedness.

Mandates the council evaluate the program and submit a report to the Legislature by January 1, 2026, assessing greenhouse gas emissions reductions, participation rates, and optimal models for maximizing low-income participation.

The program and all related provisions automatically repeal on January 1, 2027.
