Requires the counsel for committed individuals, local law enforcement leadership, county counsel, and district attorneys to provide assistance and consultation in locating and securing housing for sexually violent predators being conditionally released, with the department convening a committee for this purpose.

Authorizes courts to order status conferences to evaluate the department's progress in securing housing and obtaining required assistance and consultation, with authority to sanction participants for failure to appear without good cause.

Restricts courts from finding "extraordinary circumstances" to place individuals outside their county of domicile unless the domicile county petitions the court and demonstrates an exhaustive housing search with meaningful participation from required parties.

Requires the domicile county to provide at least one alternative placement county for consideration before a court can find extraordinary circumstances, with noticed hearings allowing the department and alternative county district attorney to be heard.

Mandates the Judicial Council report annually to the Legislature on instances where courts issue extraordinary circumstances findings, including the court's stated grounds for such findings.
