Authorizes the Metropolitan Transportation Commission to apply to the California Transportation Commission for high-occupancy toll facilities on State Route 37, creating the SR-37 Toll Authority to operate tolling infrastructure and collect tolls on the Sonoma Creek Bridge.

Establishes toll rates between $1 and the Bay Area Toll Authority's maximum rates (excluding SF-Oakland Bay Bridge peak rates), effective January 1, 2026, with tolls to cease after bond repayment unless extended by two-thirds authority vote.

Requires 50% discount for qualifying high-occupancy vehicles and 25-50% discount for low-income drivers subscribing to electronic toll collection; toll revenues directed to the SR-37 Toll Account for capital improvements, transit expansion, ecological restoration, and bond obligations.

Authorizes the authority to issue toll bridge revenue bonds backed by toll revenues and to develop an expenditure plan updated every three years, with administrative costs capped at 3% of toll bridge revenues.

Specifies toll revenues may fund near-term improvements between State Route 121 and Mare Island and long-term sea level rise adaptation projects rebuilding State Route 37 as a multimodal facility from Novato to Vallejo.
