Establishes a regional pilot program in Santa Clara, San Mateo, and San Francisco counties until January 1, 2030 to test repository and distribution programs for surplus medication donated to uninsured patients.

Expands eligible donor organizations to include facilities licensed by the State Department of Social Services, Department of Public Health, Department of Health Care Services, Department of Corrections and Rehabilitation, and Division of Juvenile Justice.

Allows any California-licensed pharmacy not on probation to donate unused, unexpired medication, removing previous restrictions that limited donations to certain pharmacy types.

Permits donated medication to be transferred multiple times within a county and from multiple facilities to be commingled, with requirements to track medication origins and maintain detailed records.

Requires participating entities to report medication errors to the Board of Pharmacy within 30 days and mandates the Board to submit an evaluation of the pilot program to the Legislature by January 1, 2028.
