Establishes the Unincorporated Area Annexation Incentive Program within the Office of Planning and Research to provide grants to cities for infrastructure projects related to annexing substantially surrounded unincorporated areas.

Allows grants for municipal infrastructure including roads, street lighting, sidewalks, stormwater management, parks, and greenways, with priority given to fully surrounded areas, public health and safety projects, and disadvantaged communities.

Requires the office to match city contributions on a dollar-for-dollar basis up to a maximum threshold determined by the director.

Requires cities to submit applications including descriptions of the unincorporated area, proposed infrastructure improvements with estimated costs, and a funding plan for construction and maintenance.

Mandates the Office of Planning and Research develop implementation guidelines by September 1, 2023, in consultation with local agency formation commissions, counties, cities, and residents of disadvantaged unincorporated communities.
