Authorizes the Board of Supervisors of Sacramento County to establish a health authority to manage Medi-Cal managed care plan procurement and operations in the county.

Requires the health authority to meet with health plans intending to contract with the state and designate at least 2 Knox-Keene licensed health plans to the Department of Health Care Services for approval based on specified criteria.

Permits the health authority to pursue establishing its own Knox-Keene licensed health plan or negotiating with a licensed health plan to serve as the county-sponsored local initiative to provide Medi-Cal services.

Allows the health authority to meet with existing Medi-Cal managed care plans to review and discuss strategies for improving quality, cost, and access of Medi-Cal services until a county-sponsored local initiative is implemented.

Specifies that the Department of Health Care Services retains final approval authority and may determine alternative health plans if the health authority fails to designate at least 2 department-approved plans.
