Requires the Department of Health Care Services to develop standard forms by January 1, 2022 for Medi-Cal specialty mental health services under the Early and Periodic Screening, Diagnostic, and Treatment Program, including intake, assessment, and treatment planning forms.

Mandates the department consult with county behavioral health directors, county associations, children's hospitals, foster youth advocates, and community-based providers when developing the standard forms.

Authorizes the department to develop and maintain a list of department-approved nonstandard forms that counties may use in addition to the standard forms.

Requires the department to conduct regional trainings by July 1, 2023 for county mental health plan personnel and providers on proper completion of standard forms, with counties distributing training materials to their provider networks.

Mandates all county mental health plan contractors begin exclusively using the standard forms (or approved nonstandard forms) by July 1, 2023 for intake, assessment, treatment planning, eligibility determination, and reimbursement determinations.
