Requires health care providers who decline to participate to inform patients of their non-participation, document the request date, and transfer relevant medical records upon request.

Mandates health care entities post their medical aid-in-dying policies on public websites and prohibits false, misleading, or deceptive practices regarding willingness to participate.

Extends the End of Life Option Act sunset date from January 1, 2026 to January 1, 2031.
