Creates a pilot program from January 1, 2023, to January 1, 2026, requiring the Department of Housing and Community Development to provide housing rental-related subsidies to skilled nursing facility patients who would otherwise be discharged due to lack of housing.

Authorizes the department to administer subsidies directly or contract with qualified local government agencies or nonprofit organizations to manage the program.

Requires the department to adopt guidelines establishing criteria for subsidies, including eligibility requirements, income limits, and subsidy amounts.

Makes the department subject to the Administrative Procedure Act when adopting regulations to implement and administer the program.

Program becomes operative on January 1, 2023, and is repealed on January 1, 2026, unless extended by the Legislature.
