Establishes the Dementia Care Navigator Grant Pilot Program administered by the California Department of Aging in partnership with organizations using community health workers, promotores, and health navigators.

Authorizes grants on a competitive basis with priority given to organizations serving underserved communities to provide dementia care navigation training services including personalized care planning, family education and support, clinical trial information, and assistance with safety, legal, financial, and transportation planning.

Requires grant-receiving organizations to submit annual reports by December 31 detailing number of individuals served, demographic data, and staff trained, with ineligibility for future grants if reports are not provided.

Requires the California Department of Aging to report to the Legislature by December 1, 2027, on organizations that received grants and services provided.

Makes the pilot program inoperative on July 1, 2027, with full repeal on January 1, 2028; implementation contingent upon appropriation.
