Requires the Department of Health Care Services to offer contracts to health care service plans for Highly Integrated Dual Eligible Special Needs Plans (HIDE-SNPs) and Fully Integrated Dual Eligible Special Needs Plans (FIDE-SNPs) beginning no later than January 1, 2025

Dual eligible beneficiaries enrolled in HIDE-SNPs or FIDE-SNPs may select from multiple available plan options with high Medicare star ratings and may maintain their established or selected health care providers

Contracting plans must perform all required care coordination and data-sharing functions and provide documentation demonstrating care integration for dual eligible beneficiaries

Applies to individuals age 21 and older who are enrolled in Medicare Part A or Part B and are eligible for Medi-Cal benefits

Addresses California's 1.6 million dual eligible beneficiaries who account for 34 percent of total Medicare expenditures despite representing only 20 percent of the Medicare population
