Establishes a case management services pilot program under the California Department of Aging, operating until January 1, 2030, to provide services helping older individuals maintain health and economic stability.

Requires Alameda, Marin, and Sonoma counties to participate in the pilot program and coordinate with their respective area agencies on aging or other community-based entities to deliver case management services.

Mandates participating counties to measure performance outcomes and submit annual reports to the department for the first five years of appropriation to determine program efficacy and inform the master plan for aging.

Provides that if the Commission on State Mandates determines this bill creates state-mandated local program costs, reimbursement shall be made pursuant to existing statutory procedures in Government Code Part 7.
