Expands the California Perinatal Equity Initiative to include city health departments alongside county health departments as eligible recipients of state funding.

Authorizes the State Department of Public Health to allocate funds to up to 15 local health jurisdictions to establish Community Centers of Excellence and implement evidence-based interventions to reduce Black infant mortality.

Requires participating local health jurisdictions to implement at least two of five specified intervention strategies, including group prenatal care, preconception care, fatherhood initiatives, home visitation programs, or locally-determined evidence-based strategies combining social and medical interventions.

Permits funded jurisdictions to provide technical assistance, conduct public awareness efforts, participate in statewide learning collaborations, and collect data on program outcomes related to Black infant health.

Mandates that allocated funds supplement rather than replace existing funding for infant health equity programs, and requires consultation with stakeholders including health departments, providers, advocates, and community representatives.
