Authorizes counties to establish mental health multidisciplinary personnel teams to identify, assess, and link justice-involved persons diagnosed with mental illness to supportive services while incarcerated and after release from county jail.

Allows team members from provider agencies (including mental health, social services, probation, law enforcement, and homeless services) to share confidential information for coordinating supportive services and ensuring continuity of care.

Requires each county to develop protocols governing information sharing that comply with privacy laws including HIPAA, HITECH, and California's Confidentiality of Medical Information Act, and must provide protocols to the State Department of Health Care Services.

Designates team members to maintain the same privacy and confidentiality obligations as the person disclosing information, with violations subject to existing civil and criminal penalties.

Requires participating agencies to implement security controls, confidentiality training, uniform written policies, and data safeguards to protect shared information from unauthorized access or disclosure.
