Authorizes San Mateo County to expand homeless adult and family multidisciplinary personnel teams to include identification and services for individuals at risk of homelessness, joining Los Angeles, Orange, Riverside, San Bernardino, San Diego, Santa Clara, and Ventura counties.

Defines "individual at risk of homelessness" as someone who is indigent or receiving public benefits and has recently exited detention/treatment facilities or aged out of child welfare/juvenile justice systems within the past 12 months, plus presents with significant health, mental health, or substance use issues.

Requires multidisciplinary teams to include persons trained in homelessness prevention and allows team members to share confidential information to identify at-risk individuals and coordinate housing and supportive services for up to 30 days or until individual opts out.

Mandates that teams notify individuals at risk of homelessness that their confidential information may be shared for coordinating services and must attempt to obtain their consent before sharing.

Removes the January 1, 2025 repeal date, making these provisions permanent rather than temporary.
