Requires health care service plans and health insurance policies issued, amended, or renewed on or after January 1, 2027, to cover and reimburse providers for social determinants of health screenings addressing food insecurity, housing insecurity, and transportation needs.

Mandates providers use standardized codes recognized by the Centers for Medicare and Medicaid Services when documenting patient responses to screenings and requires use of validated screening tools from nationally recognized organizations.

Requires health care service plans and health insurers to provide primary care physicians with adequate access to peer support specialists, lay health workers, social workers, and community health workers in counties where they have enrollees or insureds.

Makes social determinants of health screenings a covered benefit for Medi-Cal beneficiaries with reimbursement provided by the state department or Medi-Cal managed care plans, including separate reimbursement for federally qualified health centers and rural health clinics.

All provisions are contingent upon legislative appropriation and the Department of Managed Health Care and Department of Insurance are authorized to adopt guidance until regulations are formally adopted.
