Authorizes associations of employers established before January 1, 1966 to offer large group health care service plans to small group employer members through multiple employer welfare arrangements (MEWAs), provided they meet specified requirements including coverage of engineering, surveying, or design industry employees.

Requires associations and MEWAs to register with the Department of Managed Health Care by June 1, 2025 and annually file evidence of ongoing compliance with regulatory requirements.

Prohibits health care service plans from marketing, issuing, amending, renewing, or delivering large employer coverage to associations or MEWAs on or after June 1, 2025 unless they are registered with the department and in compliance with requirements, or have pending registration applications.

Requires the Department of Managed Health Care to conduct an analysis of MEWAs' impacts on California's small employer health insurance market and post a report on its website by July 1, 2026; requires health care service plans and insurers to provide information for the analysis.

Repeals all provisions on January 1, 2030.
