Requires health care service plans and health insurance policies issued or renewed on or after January 1, 2025, to cover evaluation and treatment for perimenopause and menopause without utilization management.

Mandates coverage include at least one option in each formulation and administration method for FDA-regulated systemic hormone therapy, nonhormonal medications for menopause symptoms, genitourinary syndrome treatments, and osteoporosis medications.

Requires health care service plans and insurers to annually provide current clinical care recommendations for hormone therapy from the Menopause Society or other nationally recognized professional associations to contracted primary care providers.

Allows treating providers to adjust drug doses consistent with clinical care recommendations and requires coverage be provided without discrimination based on gender expression or identity.

Excludes specialized health care service plan contracts and specialized health insurance policies from the coverage requirements.
