Health care service plans and health insurance policies issued, amended, or renewed on or after January 1, 2025, must cover cranial prostheses (wigs or hairpieces) for individuals with permanent or temporary medical hair loss.

Coverage requires a licensed provider to prescribe the cranial prosthesis for treatment of diagnosed conditions including alopecia areata, alopecia medicamentosa, scarring alopecia, and lupus.

Coverage is limited to once every 12 months per individual and capped at $750 per instance, subject to applicable cost-sharing requirements.

Medi-Cal must provide cranial prostheses as a covered benefit starting January 1, 2025, under the same requirements regarding provider prescription, frequency, and dollar limits.

Exemptions apply to specialized health care service plans and specialized health insurance policies.
