Requires health care service plans and health insurers to include coverage of durable medical equipment, services, and repairs as part of rehabilitative and habilitative services if prescribed or ordered by a licensed health professional.

Defines durable medical equipment as devices designed for repeated use to help individuals acquire, improve, maintain, or learn skills and functioning for daily living, including replacement devices and necessary fittings or adjustments.

Prohibits coverage of durable medical equipment and services from being subject to financial or treatment limitations, including annual caps or restrictions limiting coverage to home use only.

States that durable medical equipment coverage is necessary to comply with federal nondiscrimination requirements under the Affordable Care Act and is not subject to state cost defrayal obligations.

Applies to individual and small group health care service plan contracts and health insurance policies issued, amended, or renewed on or after January 1, 2017.
