Requires health facilities to include anonymous reporting options in patient safety event reporting systems

Mandates analysis of patient safety events by sociodemographic factors (age, race, ethnicity, gender identity, sexual orientation, language, disability status, payor, and sex) to identify and address disparities

Requires health facilities to implement processes for addressing racism and discrimination, including monitoring disparities and developing interventions to remedy known disparities

Requires health facilities to submit patient safety plans to the department's licensing and certification division beginning January 1, 2026, and biannually thereafter, with plans made publicly available online

Authorizes the department to impose fines up to $5,000 for failure to adopt, update, or submit patient safety plans, and allows automatic 60-day extensions for biannual submissions
