Expands colorectal cancer screening coverage to include tests assigned grades A or B, or equivalent, by accredited or certified guideline agencies approved by the California Health and Human Services Agency, in addition to the United States Preventive Services Task Force recommendations.

Requires health care service plans and health insurance policies issued, amended, or renewed on or after January 1, 2022, to provide coverage without cost sharing for qualifying colorectal cancer screening tests.

Mandates coverage without cost sharing for required colonoscopies performed following a positive result on a qualifying colorectal cancer screening test.

Allows health plans and insurers with out-of-network providers to impose cost-sharing requirements for screening services delivered by out-of-network providers.

Applies to all health care service plan contracts and health insurance policies except specialized plans.
