Remove requirement that voters provide a statement under penalty of perjury when requesting a replacement vote by mail ballot; instead require only a replacement ballot request.

Require voters requesting a replacement ballot to provide personal identifying information matching their voter registration record, including first and last name, residence address, and date of birth.

Require elections officials to advise replacement ballot requesters that requesting a ballot as someone other than the registered voter is a criminal offense.

Maintain existing procedures for voters' authorized representatives to request replacement ballots on behalf of voters, which continue to require a signed written request under penalty of perjury and signature verification.

Establish that if the Commission on State Mandates determines the bill creates state-mandated costs, reimbursement shall be made under the statutory procedures in Government Code Part 7.
