Amends Health and Safety Code Section 103871 to require the State Department of Public Health to collect data on neurodegenerative disease incidence in California until January 1, 2028.

Changes the definition of "neurodegenerative disease" to mandate inclusion of amyotrophic lateral sclerosis (ALS) rather than merely permit it as an optional disease to track.

Requires hospitals, facilities, physicians, and health care providers diagnosing or treating neurodegenerative disease patients to report cases to the department in a prescribed format.

Authorizes the department to access medical records related to neurodegenerative disease cases and imposes civil penalties of up to $500 per day for willful failure to grant access to records.

Specifies that neurodegenerative disease registry may also include Alzheimer's disease, multiple sclerosis, and Huntington's disease, with implementation contingent on available funding.
