Modifies the electronic application for insurance affordability programs to include an optional organ donation enrollment feature for the Donate Life California Organ and Tissue Donor Registry.

Organ donation enrollment option must include checkboxes allowing applicants to either consent to registry enrollment or decline, and must be voluntary and not a required part of the application.

Requires the department to electronically transmit weekly to Donate Life California the applicant's full name, address, year of birth, and driver's license or identification card number for those who choose to enroll.

Mandates that a specific disclosure regarding organ donation rights, restrictions, and contact information for Donate Life California be included with the enrollment option checkboxes.
