Establishes three clearance levels for program providers entering California state prisons: short-term clearance (3 or fewer days per event), annual clearance (full calendar year at one institution), and statewide program provider clearance (multiple institutions routinely).

Requires the Department of Corrections and Rehabilitation to provide standardized forms and clearance packets to institutions and prohibits institutions from requiring additional "local" forms.

Mandates the department notify all applicants of approval or disapproval decisions within 30 days of application receipt (or 30 days after receiving Department of Justice background check results) and notify all applicants of their right to appeal with final appeal decisions within 90 days.

Requires the department to designate a standardized approval process for formerly incarcerated program providers and prohibits excluding them based on institution type or location unless extraordinary circumstances exist, with denials requiring written explanation.

Authorizes program provider identification cards valid for five years that allow holders to enter specified institutions without a sponsor and to escort other authorized program providers.
