Requires health care service plans and disability insurance policies issued, amended, delivered, or renewed on or after July 1, 2024, to cover formulas for treatment of chronic digestive diseases (including Crohn's disease) and inherited metabolic disorders.

Defines "formula" as enteral products including nutritional drinks prescribed by physicians, nurse practitioners, or registered dieticians for medically necessary treatment of metabolic and digestive disorders.

Limits coverage to formulas only and excludes special food products for chronic digestive diseases, though both remain covered for phenylketonuria (PKU) treatment under existing law.

Exempts Medi-Cal managed care plans from coverage requirements to the extent services are excluded from their state contracts.

Applies to all health care service plan contracts and disability insurance policies except specialized health plans, vision-only, dental-only, and other limited benefit insurance products.
