Extends the prohibition on the Alameda Health System Hospital Authority entering into contracts to replace unionized physician and surgeon services from January 1, 2024 to January 1, 2035.

Requires the hospital authority to provide clear and convincing evidence that needed medical care can only be delivered cost effectively by another entity before privatizing physician and surgeon services.

Mandates negotiation with the recognized collective bargaining unit representing physician and surgeon employees over any privatization decision, with disputes subject to final binding arbitration.

Maintains existing provisions regarding the hospital authority's governance structure, independence from county operations, and obligations to continue all existing medical center services to Alameda County residents.
