Establishes a 12-member physician credentialing board within the California Health and Human Services Agency to streamline and standardize credentialing requirements for physicians across health care plans and insurers.

Requires the board to develop and maintain a standardized credentialing form by July 1, 2027, that all health care service plans and insurers must use for credentialing purposes; permits only requests for clarification of information on the form.

Extends credentialing timelines for all health care service plans and insurers to assess physician qualifications within 90 days of receiving a completed application and notify applicants within 10 business days of receipt (effective January 1, 2026).

Excludes Medi-Cal managed care contracts under specified welfare code chapters from the standardized credentialing form requirements.

Provides that no state reimbursement is required because any local costs result from changes to criminal penalties, not new mandated local programs.
