Authorizes individuals determined eligible for pediatric hospice services before age 21 to continue receiving those services after age 21 if certified by a physician under federal Social Security Act requirements.

Allows individuals determined eligible for pediatric palliative care services before age 21 to continue receiving medically necessary palliative care services after age 21 when determined eligible by their treating health care provider.

Requires the California Department of Health Care Services to seek necessary federal approvals and implements these provisions only if federal approvals are obtained and federal financial participation is available.

Directs the Legislature to investigate future legislation expanding access to pediatric palliative and hospice care, including requiring insurance coverage, expanding eligibility definitions, and creating programs for uninsured or underinsured children.

Established as "Sophia's Act" and approved by the Governor on October 13, 2023.
