Requires statements from any residents involved in the incident to be included in reports, with the restriction that such statements cannot be taken by custodial staff in positions of power or influence over the resident.

Establishes this as a state-mandated local program, requiring the state to reimburse local agencies and school districts for costs if the Commission on State Mandates determines reimbursement is necessary.
