Requires local registrars, county recorders, and the State Registrar to issue certified birth records without a fee to incarcerated persons in state prisons or county correctional facilities.

Allows incarcerated persons or anyone lawfully entitled to request records on their behalf to apply using an affidavit attesting to incarceration status.

Establishes verification methods for incarceration status including confirmation through Department of Corrections and Rehabilitation inmate locator websites, prison ID cards, postmark evidence, or signatures from correctional officials.

Directs the State Department of Public Health to develop a standardized affidavit for incarcerated persons to use when requesting certified birth records.

Limits requesters to one birth record per application per eligible incarcerated person and provides that the State Registrar shall provide one copy per year with discretion to provide additional copies.
