Requires health care service plan contracts and health insurance policies issued, amended, or renewed on or after July 1, 2025, to establish reimbursement processes for community paramedicine programs, triage to alternate destination programs, and mobile integrated health programs.

Mandates that enrollees and insured individuals receiving covered services from noncontracting programs pay the same cost-sharing amounts as they would for services from contracting programs.

Prohibits reimbursement rates from exceeding the health care service plan's or health insurer's usual and customary charges for services rendered.

Makes services provided by these programs covered benefits under the Medi-Cal program, contingent on federal approvals, available federal financial participation, and legislative appropriation.

Exempts the state from local reimbursement obligations under Article XIII B of the California Constitution because violations constitute crimes under existing law.
