Requires the Department of Developmental Services to establish statewide standardized processes and procedures for the Self-Determination Program by January 1, 2026, ensuring consistent application across regional centers and measurable improvements in equitable enrollment by race, ethnicity, and regional center.

Mandates regional centers collect and report race, ethnicity, and preferred language data starting January 1, 2025, using U.S. Core Data for Interoperability categories, with individuals having the right to decline to provide this information.

Establishes standardized templates and procedures for individual program plans, respite service assessments, vendorization, and intake processes by specified dates, with regional center deviations requiring department approval.

Requires regional centers to ensure participant choice of financial management services providers is respected and that the full cost of the chosen provider is included in the participant's spending plan, subject to federal funding eligibility.

Establishes statewide and local volunteer advisory committees to oversee Self-Determination Program implementation, with majority consumer and family representation, and requires the Governor to appoint a Deputy Director of Self-Determination subject to Senate confirmation.
