Requires insurance institutions and agents to provide a clear and conspicuous privacy notice to customers at least annually during the customer relationship, with the 12-month period defined consistently by the institution or agent.

Permits privacy notices required under existing law (Section 791.04) and this new requirement to be combined into a single notice or provided separately, as long as both sets of requirements are met.

Allows insurance institutions or agents to comply with abbreviated notices if they do not disclose medical, personal, or privileged information to nonaffiliated third parties and inform consumers of their rights to access, correct, amend, or delete personal information within 30 business days.

Requires notices to inform consumers of their right to file a statement disagreeing with the insurance institution's refusal to fulfill a request to access, correct, amend, or delete personal information.

Exempts insurance institutions and agents from providing annual notices to former customers with whom they no longer have a continuing relationship.
