Caps out-of-pocket costs for insulin at $35 for a 30-day supply for health care service plan contracts and disability insurance policies issued, amended, or renewed on or after January 1, 2024 (or January 1, 2025 for individual/small group market policies).

Requires coverage of at least two insulins for each insulin drug type in all dosage forms and concentrations, or one insulin if only one exists for that drug type.

Prohibits high deductible health plans from imposing deductibles, coinsurance, or cost-sharing on insulin prescription drugs unless doing so would conflict with federal requirements.

Maintains existing requirements for coverage of diabetes management equipment and supplies, including blood glucose monitors, insulin pumps, lancets, and diabetes self-management education and training.

Exempts certain insurance types including vision-only, dental-only, Medicare supplement, and long-term care insurance from these requirements.
