Replaces the term "intersexuality" with "variations in sex characteristics/intersex status" throughout the Lesbian, Gay, Bisexual, and Transgender Disparities Reduction Act and requires the State Department of Public Health to collect this demographic data from third parties including local health jurisdictions by March 28, 2029.

Adds sexual orientation, gender identity, variations in sex characteristics/intersex status, and sex assigned at birth to the information that health care providers must disclose to immunization information systems, with restrictions prohibiting disclosure of this information for patients under 18 years of age.

Requires the State Department of Public Health to prepare and annually post an online report and submit to the Legislature concerning SOGISC data collection efforts, compliance levels, data analyses performed, improvements to services for LGBTQI+ populations, and progress on implementing recommendations from a 2023 California State Auditor report.

Prohibits public disclosure of personally identifiable information that would identify individuals who provided voluntary SOGISC self-identification information, with legislative findings that this limitation is necessary to protect the safety of individuals providing such sensitive data.

Provides that if the Commission on State Mandates determines the bill creates state-mandated local program costs, reimbursement shall be made pursuant to existing statutory procedures.
