Requires all full service health care service plans and health insurers to subscribe to and use the Council for Affordable Quality Healthcare (CAQH) credentialing form starting January 1, 2028

Mandates health plans and insurers make credentialing determinations within 90 days of receiving a completed provider application, with notification of receipt within 10 business days

Grants automatic 120-day provisional approval if the 90-day deadline is not met, unless the applicant faces licensing discipline, has adverse action reports or malpractice payments in the National Practitioner Data Bank, or hasn't been credentialed by that plan in the past five years

Requires activation and notification within 10 days of successful credentialing approval

Exempts Medi-Cal managed care plan contracts from these credentialing timeline requirements
