Enacts the Forensic Accountability, Custodial Transparency, and Safety (FACTS) Act of 2025, prohibiting sheriff-coroners in counties with combined sheriff-coroner offices from determining the cause, manner, and circumstances of in-custody deaths, effective January 1, 2027

Requires sheriff-coroners to contract with independent coroner offices from other counties, county medical examiners, or private third-party medical examination providers to investigate in-custody deaths

Defines "in-custody death" to include deaths of persons detained, under arrest, being arrested, en route to incarceration, or incarcerated in municipal jails, county jails, state prisons, federal correctional facilities, or immigration detention facilities

Mandates county boards of supervisors to annually enter into service agreements with independent examiners, and prohibits contracted private providers from performing non-custody death examinations for that county during the agreement term

Requires examination findings to be delivered to the sheriff-coroner, district attorney, county health officer, board of supervisors, and the decedent's next of kin
