Establishes new licensing requirements for pharmacy benefit managers (PBMs) contracting with health plans or insurers, effective January 1, 2027, including application fees up to $25,000, financial reporting requirements, and creation of the Pharmacy Benefit Manager Fund for oversight activities

Restricts Medi-Cal eligibility for individuals 19+ without satisfactory immigration status who apply on or after January 1, 2026 to only pregnancy-related services and emergency medical treatment, and imposes a $30 monthly premium for certain non-citizen beneficiaries ages 19-59 beginning no sooner than July 1, 2027

Reinstates consideration of resources/assets for non-MAGI Medi-Cal eligibility determinations starting January 1, 2026, with a disregard threshold of $130,000 for single-member households plus $65,000 for each additional member up to 10

Authorizes the State Department of Public Health to spend up to $75 million from the AIDS Drug Assistance Program Rebate Fund, including $65 million to replace federal HIV/AIDS funding that has been reduced or eliminated due to federal policy actions

Extends the Enhanced Treatment Program (ETP) for acute psychiatric hospitals operated by the State Department of State Hospitals until January 1, 2030, and requires emergency regulations by January 31, 2026 establishing staffing standards specific to acute psychiatric hospitals not operated by state hospitals
