Requires the California Department of Corrections and Rehabilitation to notify county probation departments of a prisoner's scheduled release date and relevant information at least 90 days before release, up from the previous 30-day requirement

When a discharge date is set or reset with fewer than 90 days notice, the department must provide notification within 5 business days, but no later than 30 days before release

Mandates the department provide county probation with contact information for prerelease, postrelease, and enhanced care managers to integrate CalAIM (Medi-Cal) processes with local reentry services and court-ordered conditions

Requires coordination between the Department of Corrections and county probation when a person's current county of residence differs from their last legal residence, including developing plans for release and transport

Applies to persons released from prison subject to postrelease community supervision for up to 3 years, excluding those convicted of serious felonies, violent felonies, or classified as high-risk sex offenders
